Initial comprehensive preventive medicine evaluation and management

Periodic comprehensive preventive medicine reevaluation and management

These codes include care for preexisting conditions or minor problems, and exclude behavior change interventions (99406–99409). Offices often call these visits physicals, well checks, annual exams or routine exams.

Risk factor and behavior change counseling

  • 99401–99405: preventive counseling or risk factor reduction at an encounter separate from the preventive exam, such as family problems, diet and exercise
  • 99406–99407: smoking and tobacco cessation counseling
  • 99408–99409: alcohol or substance abuse structured screening

Medicare

Medicare doesn't cover these preventive medicine codes. For behavioral counseling for obesity, CMS recognizes two HCPCS codes:

G0447
Face-to-face behavioral counseling for obesity, 15 minutes
G0473
Face-to-face behavioral counseling for obesity, group (2–10), 30 minutes
